Free athletic physicals for students at DMS will be held at at Stone Mountain Health Services, 306 S. Shady Ave., on June 24 from 9:00 am- 3:00 pm. Please follow the schedule below.
IMPORTANT: There is a new VHSL Physical Form this year. Athletes will need to complete forms from the doctor's office AND the VHSL Physical Form. These forms will not be accepted if they are printed front to back. DMS has the forms you can pick up prior to June 24. Please make sure all signatures are completed and that all pages are completely filled out.
9:00 to 10:30 RISING JUNIORS AND SENIORS
10:30 to 12:00 RISING FRESHMAN AND SOPHOMORES
1:00 to 2:00 RISING 6TH, 7TH, 8TH GRADES
2:00 TO 3:00 OPEN TIME FOR THOSE UNABLE TO MAKE EARLIER SLOTS
